The statement needs to be filled if your student card and/or the validating sticker gets lost or stolen.
In Neptun please choose ’Reason for request: Lost’ in your online student card application (Administration/Student Card Request).
If you had a validating sticker for the actual semester on the lost student card, (1) you fill the Statement (2) pay HUF 3,500 service fee. This fee can be transcribed in Neptun/Finances/Overview/New item and can get fulfilled immediately via online bank card payment in Neptun:

In case of loss, the student card request is forwarded only if (1) the completed Statement is received by the Student Card Administration Unit and (2) the HUF 3,500 service fee is paid.
Student Card Administration Units are located at the Faculties for the students of:
- Faculty of Humanities and Social Sciences (BTK)
- Faculty of Health Sciences (ETK)
- Faculty of of Cultural Sciences, Education and Regional Development (KPVK)
- Faculty of Engineering and Information Technology (MIK)
- Faculty of Art (MK)
- Faculty of Sciences (TTK)
- Doctoral School of the Medical School (ÁOK)
- Doctoral School of the Faculty of Pharmacy (GYTK)
- and for the students participating in teacher education degree programs, it is managed by the Teacher Training Division (KTA - Közismereti Tanárképzési Alközpont)
UP Central Registrar’s Office (KTI) handles the student card administration of the Faculty of Law (ÁJK), the Medical School (ÁOK), the Faculty of Pharmacy (GYTK), and the Faculty of Business and Economics (KTK).
